Instead of appending all pages of file B after file A, the tool takes page 1 from A, then page 1 from B, then page 2 from A, then page 2 from B, and so on — producing an interleaved document.

The most common reason is double-sided scanning. If your scanner only does one side at a time, you scan all odd (front) pages first, then flip the stack and scan all even (back) pages — then use this tool to interleave them back into reading order.

If file A has more pages than file B (or vice versa), the remaining pages from the longer file are appended at the end of the interleaved sequence.

Many single-pass scanners produce back-side pages in reverse order. Reverse that second PDF first (using a PDF reverse tool), then run the alternating merge.

A regular merge appends all pages of file A, then all of file B. Alternating merge interleaves them page-by-page, producing A1, B1, A2, B2 instead of A1, A2, B1, B2.
